Here are the forms you'll need for requesting assistance from the PTA.

Mini Grant: From October 1 to January 31, on approval, receive $125 per staff member for use in the classroom. Fill out this form, include the requested documentation, and put it in the PTA box located in the office. Completed forms will be reviewed and approved each month at the PTA meeting.
